Community Health Workers

A Community Health Worker (CHW) is a frontline public health professional who helps individuals and communities access healthcare, understand their health needs, and navigate social services. CHWs typically come from—or have deep knowledge of—the communities they serve, which helps them build trust and provide culturally appropriate support.

The Work of a CHW

A Community Health Worker May:

Provide health education (nutrition, chronic disease prevention, maternal health)

Help people navigate the healthcare system (appointments, insurance, referrals)

Conduct outreach in homes, clinics, schools, or community centers

Connect individuals to social services such as housing, food assistance, or transportation

Advocate for community health needs

Support patients in managing chronic conditions

Assist with care coordination and follow-up
